How to obtain an Vote by Mail Ballot

The first day to request a Vote by Mail ballot for the Consolidated General Election is Wednesday January 4, 2017. The first day Vote by Mail ballots will be mailed is Thursday March 30, 2017. 

By Mail

The last day we can mail you a ballot is 5 days prior to the election. The ballot needs to be postmarked by Election Day and must be in our office within two weeks after the election.

Please note that if you will be out of the country for the election, you must complete and return an FPCA form.

NOTE: ELECTION MAIL CANNOT BE FORWARDED

Incapacitated/Disabled Voters

Illinois State Statute allows voters who are incapacitated to complete an Application for Absentee Voter's 5 Year Identification Card form* along with an Affidavit of Attending Physician* in order to automatically be sent a Vote by Mail ballot application for every election within five years.
